What are full-arch dental implants?

When there are no teeth left in a jaw, the whole arch can be rebuilt on implants. Implants are artificial tooth roots placed in the jawbone; once they have integrated, a prosthesis is attached to them. At Miško Clinic there are two options:

  • A fixed prosthesis on 4 to 6 implants. This approach is often called All-on-4 or All-on-6. The prosthesis is attached to the implants, so you do not take it out each day. Our price list includes fixed prostheses on implants in metal-ceramic and in zirconia.
  • A removable denture on two implants. Locator attachments on the implants hold the denture firmly in place. This option is considered when an ordinary denture does not stay in place well.

How many implants you need, whether there is enough bone and which option suits you are decided after a consultation and a 3D CBCT scan. If bone is lacking, bone augmentation or a sinus lift may be needed. The surgical stage is carried out by oral and maxillofacial surgeon Donatas Kubilius, and the clinic's dentists complete the prosthetic stage.

Who are full-arch implants for?

  • A jaw with no teeth left: 4 to 6 implants can hold a fixed prosthesis (All-on-4, All-on-6).
  • A denture that will not stay in place: 2 implants with locators hold a removable denture firmly.
  • Remaining teeth that can no longer be saved: whether a tooth is still worth treating is decided after an examination. Our article “Save a Tooth or Have It Removed?” explains how that decision is made.
  • You would prefer fixed teeth to a removable plate: a fixed prosthesis is attached to the implants and is not taken out each day.
  • There is enough bone, or it can be built up: this only becomes clear after a 3D CBCT scan.

Full-arch implant treatment step by step

    Consultation and 3D CBCT scan

    We examine your mouth, talk through what you hope for and assess the bone and the position of the sinuses and nerves. Then we decide whether a fixed or a removable option suits you and how many implants will be needed.

  2. 2

    A written treatment plan

    The plan sets out the stages, materials and prices. It stays valid for several months, and treatment can be split into stages and paid for after each visit.

  3. 3

    Preparing the mouth

    If needed, teeth that cannot be saved are removed, and bone augmentation or a sinus lift is carried out. Bone augmentation and sinus lifts are performed by surgeon Donatas Kubilius.

  4. 4

    Placing the implants

    Oral and maxillofacial surgeon Donatas Kubilius places the implants under local anaesthetic. Their number and positions are planned in advance from the 3D CBCT scan.

  5. 5

    Healing and osseointegration

    The implants fuse with the bone. How long this takes depends on the jaw and on whether bone was added (see our implant healing article). The temporary solution you can wear in the meantime is agreed when we plan your treatment.

  6. 6

    Impressions and making the prosthesis

    Once the implants have integrated, impressions are taken and a dental technician makes the fixed or removable prosthesis in the chosen material.

  7. 7

    Fitting, fixing and follow-up

    We check the bite, shape and comfort and adjust if needed, then fix the prosthesis or hand over the removable denture. We explain how to look after it and plan your follow-up visits.

Who carries out this treatment

Caring for teeth on implants

After implant surgery, follow the surgeon's instructions. Our article “Dental Implant Healing” explains what to expect in the first days, weeks and months, how to look after the area and when to call us.

Once the prosthesis is in place, daily cleaning and regular check-ups protect the implants:

  • Clean a fixed prosthesis and the area around the implants every day with a soft or electric toothbrush and interdental brushes with plastic-coated wire, plus a water flosser if your dentist recommends one.
  • Take a removable denture out and rinse it every day. You will find general denture care advice on our dental prosthetics page.
  • Attend professional dental hygiene visits every 6 months and a dental check-up once a year.
  • If you grind your teeth, ask about a protective night guard (see bruxism treatment).
  • If you smoke, tell your dentist: smoking raises the risk of implant failure and of inflammation around the implant.

Contact the clinic without delay if an implant or the fixed prosthesis starts to feel loose, if swelling or pus appears around an implant, or if you have a fever. If swelling makes breathing or swallowing difficult, call 112.

How much do All-on-4 implants cost?
Most of the cost is the implants and the prosthesis. According to our price list, one implant costs 550 € (Osstem) or 650 € (Straumann Roxolid SLA), so four implants come to 2,200 € or 2,600 €. A fixed prosthesis on implants costs 3,500–5,000 € in metal-ceramic or 4,000–6,000 € in zirconia. The consultation, the 3D CBCT scan and, if needed, healing abutments (Osstem 70 €, Straumann 80 € each), extractions, bone augmentation or a sinus lift are charged separately. What is the difference between a fixed and a removable prosthesis on implants?
A fixed prosthesis is attached to 4 to 6 implants and is not taken out each day, so it needs careful cleaning in the mouth. A removable denture sits on two implants with locators, which hold it firmly, yet you can take it out to clean it. The removable option costs less (900 €, plus the implants), while a fixed prosthesis costs 3,500–5,000 € in metal-ceramic or 4,000–6,000 € in zirconia. Which suits you depends on your bone, your bite and your preferences, and we discuss this at the consultation.
How many implants are needed to replace all the teeth?
4 to 6 implants can hold a fixed prosthesis (All-on-4 or All-on-6), while 2 implants with locators hold a removable denture firmly. We decide the exact number and positions after a 3D CBCT scan, once we have assessed the bone and the position of the sinuses and nerves.
Can teeth be replaced with implants if bone is lacking?
A lack of bone does not automatically rule implants out: whether there is enough bone, or whether it needs building up, is decided after a 3D CBCT scan. If needed, surgeon Donatas Kubilius carries out bone augmentation and, in the upper jaw, a sinus lift (open sinus lift surgery costs 500 €). Bone augmentation is not listed in the price list, so its cost is discussed individually at the consultation. In that case healing takes longer.
How long does the whole treatment take?
It depends on whether teeth need removing or bone needs adding, and on which jaw the implants go into. The final prosthesis is made once the implants have integrated. Our article on implant healing notes that an implant usually integrates in about 3 months in the lower jaw and in 4 to 6 months in the upper jaw, while after bone augmentation or a sinus lift the total time may reach 9 to 12 months. Will I be without teeth while the implants heal?
While the implants integrate you can wear a temporary solution. What it will be in your case depends on your mouth and the chosen plan, so we agree it when drawing up the treatment plan.

Yes. A larger treatment plan can be split into stages and paid after each visit. After the consultation you receive a written plan with prices; it stays valid for several months, so there is no need to decide on the day.
