What is dental implants in?

A dental implant is a titanium screw that replaces the natural root of a tooth. It is placed into the jawbone, fuses to it over a few months (osseointegration) and becomes the foundation for a crown, a bridge or a full denture. An implant lets you replace a missing tooth without needing to grind down the neighbouring teeth, as would happen with a bridge.

At Miško Clinic we choose classic implantology, not the rushed 'teeth in a day' approach. After a tooth is removed we wait on average 4 months for good-quality bone to form, and only then place the implant.

Once the implant has integrated we wait up to 4 months again, and only then fit the crown. This path is slower but far more reliable in the long term.

The surgical part (placing the implant itself) is carried out by oral and maxillofacial surgeon Donatas Kubilius, who has extensive experience in this field. In complex cases (lack of bone, sinus lift) he performs bone augmentation.

The crown stage is completed by Dr Lauras Smilgevičius. We use two trusted implant makers: Osstem (South Korea) and Straumann (Switzerland).

When is it needed?

  • One lost tooth: you want to replace it without grinding down its healthy neighbours.
  • Several missing teeth in a row: implants hold a bridge that feels like natural teeth.
  • A fully toothless jaw: 4 to 6 implants can hold a fixed denture (All-on-4, All-on-6).
  • A loose removable plate: 2 implants with locators hold it firmly in place.
  • A damaged tooth that cannot be saved: extraction with delayed implant placement.
  • You want to avoid a bridge: so the neighbouring teeth stay healthy and untouched.
  • There is enough bone, or it can be grown: decided after a 3D CBCT analysis.

How the procedure works

    Consultation and 3D CBCT scan

    We assess the bone, the position of the sinuses and nerves, and plan the position and type of implant.

    Extraction (if needed) and bone healing

    After a tooth is removed we wait about 4 months for good-quality bone to form for the implant.

    Placing the implant

    The procedure is carried out under local anaesthetic by surgeon Donatas Kubilius. It takes about an hour.

    Osseointegration

    Over 3 to 4 months the implant fuses with the bone. During this time you can wear a temporary solution.

    Fitting the healing abutment

    The gum is opened and a healing abutment is fitted so a natural gum contour forms.

    Impressions and crown making

    Digital impressions, and the crown is made in the laboratory in 1 to 2 weeks.

    Fitting the crown

    A zirconia ceramic crown is fixed onto the abutment, and the bite and aesthetics are checked.

Aftercare following dental implants

For the first few hours after implant placement, use a cold compress on the outside to reduce swelling. On average swelling peaks on day 2 or 3 and settles on its own within 5 to 7 days. Any pain is mild and controlled with simple medicines (ibuprofen, paracetamol).

  • Do not chew on the operated side during the first week.
  • Eat soft, lukewarm food and avoid anything hot or spicy.
  • Do not rinse vigorously for the first day; from the second day use a gentle antiseptic rinse (chlorhexidine).
  • Keep brushing your teeth, but avoid the operated area for the first days, then clean it very gently.
  • Avoid smoking for at least 2 weeks: it significantly impairs implant healing.
  • Postpone exercise for 1 to 2 weeks.
  • Take antibiotics and other medicines as instructed.

Over 3 to 4 months the implant integrates into the bone. After that you return for the crown to be made.

Sometimes I get the impression that, for patients, dental implantology is one of the most mystified areas of dentistry. On one hand, aggressive marketing creates the impression that only implants can save the world and that bridges are already outdated. On the other hand, people still carry many fears about this field: it may not integrate, or the body may reject it. So when it comes to choosing the best treatment, there is a lot of discussion and doubt.

In reality it is both simpler and less simple than that. Everything depends on the situation. Every case is different, and only during a consultation do we weigh up all the pros and cons together with the patient, until we find the most suitable and sensible treatment for them, not for the chap in the advert or the neighbour down the hall.

I do not believe in the express implantology that shouts 'I pull the tooth and place the implant the same day'. It may sound attractive from a marketing point of view, but let us leave that to the businesspeople. From a clinician's position, anticipating possible complications, it is a risky path. We choose classic implantology: better slower but reliable. So after an extraction we wait on average about four months for good-quality bone to form, and only then place the implant.

Are dental implants painful?
This is one of the most common questions and entirely natural. During the procedure we use local anaesthetic, so you will not feel any pain. After the surgery there may be some tenderness or swelling, but it is all manageable with mild medicines. And we are always close by: monitoring, caring and answering all your questions.
How much do dental implants cost?
An implant with a crown at Miško Clinic: Osstem 550 € + crown 500 € (about 1050 € in total), Straumann 650 € + crown 550 € (about 1200 € in total). Possible additional costs: 3D CBCT, healing abutment 70-80 €, sinus lift 500 €. You can see all prices in the price list.
What exactly is a dental implant?
In short, it is an artificial tooth root, a small but very strong titanium screw placed into the jawbone. Once integrated, it becomes the foundation for a crown (an artificial tooth) that looks and works just like a natural tooth.
How long do dental implants take?
Placing the implant itself is a quick procedure, usually about an hour. The full process (from extraction to crown) takes 6 to 9 months because of the necessary bone formation and osseointegration.
How long do implants last?
With proper care, a very long time, even decades. Some people live with the same implants for the rest of their lives. The key is to attend the dentist regularly and maintain good oral hygiene.
Will the implant look natural?
Yes, that is our goal. Before making the crown, we assess the colour, shape and proportions of your teeth so the new tooth blends into your smile as if it had always been there.
Can I have an implant if several teeth in a row are missing?
Yes. In some cases each tooth needs its own implant, but often we use bridges or All-on-4 systems, where several implants hold a whole row of teeth. We will discuss this during an individual consultation.
Which should I choose: an implant or a dental bridge?
An implant does not grind down the neighbouring healthy teeth, lasts longer and protects the bone from shrinking. A bridge is a cheaper and faster solution, but it requires grinding down two neighbouring teeth. The decision depends on the condition of the neighbouring teeth, the amount of bone and the budget. We discuss it during a consultation.
What is the difference between Osstem and Straumann implants?
Osstem (South Korea) is a reliable mid-priced system, widely used in Europe. Straumann Roxolid SLA (Switzerland) is a premium titanium-zirconium implant with an SLA surface that integrates faster and suits even thinner bone. Both are excellent solutions; for complex cases we more often choose Straumann.
Can the body reject an implant?
A true 'rejection' reaction (as with organ transplants) practically never happens, as titanium is biologically inert. However, an implant can fail to integrate due to infection, poor hygiene, smoking or insufficient bone. The success rate of modern implants is 95 to 98 percent.
Can I smoke if I have an implant?
Smoking significantly increases the risk of implant failure and peri-implantitis (inflammation of the gum around the implant). You must not smoke for at least 2 weeks after surgery, and over the long term smoking significantly shortens the lifespan of the implant. If you smoke, tell us openly and we will adapt the plan.
Can implants be placed with diabetes or osteoporosis?
Yes, but the chronic condition must be well controlled (for diabetes, HbA1c below 7 percent). If you take bisphosphonates (osteoporosis medicines), a consultation with your treating doctor is needed, as there is a risk of jaw osteonecrosis. Always tell us about all the medicines you take.
