What is root canal treatment in?
Root canal treatment (endodontics) is a procedure in which the damaged or infected pulp of the tooth (the nerve tissue) is removed, the root canals are carefully cleaned and disinfected, and they are then sealed with a special material (gutta-percha). This lets you keep a natural tooth that would otherwise have to be extracted.
At Miško Clinic we work with rotary nickel-titanium instruments, which are far more efficient and precise than older hand systems. For complex cases or retreatment we use a dental microscope that lets us see even thin, extra or calcified canals. All treatment is carried out under rubber dam, isolating the tooth from saliva and bacteria.
After successful root canal treatment a tooth often becomes more brittle, so we recommend protecting it with a crown. This ensures that the saved tooth lasts not just a few years but many.

When is it needed?
- Severe, throbbing toothache: especially at night or triggered by hot food.
- Lingering sensitivity to cold or heat: when the discomfort lasts more than a few seconds after the trigger.
- A swollen gum or abscess: a visible lump or redness of the gum next to the tooth.
- Deep decay: the decay has reached the nerve and an ordinary filling will no longer be enough.
- Tooth trauma: the nerve is damaged by a blow, the tooth darkens or starts to become loose.
- Recurrent infection: a previously treated tooth starts to ache again or an abscess appears at the root tip.
- Preparing for a restoration: when a tooth is to be crowned and the pulp is damaged.
How the procedure works

Examination and X-ray
We assess the tooth, take a dental X-ray and, in complex cases, a 3D CBCT scan to see the canal anatomy.

Local anaesthetic
The tooth and surrounding area are numbed so you feel no pain during the procedure.

Isolation with a rubber dam
The tooth is isolated from saliva and bacteria with a rubber sheet. This is the foundation of sterility.

Removing the pulp and cleaning the canals
With rotary NiTi instruments we carefully clean and shape the root canals, flushing them with antiseptic solutions.

Interim treatment (if needed)
If there is infection, calcium hydroxide is placed in the canals and the tooth is sealed temporarily for 1 to 2 weeks.

Final sealing of the canals
The cleaned canals are tightly filled with gutta-percha and sealer, and a control X-ray is taken.

Restoring the tooth
The crown part is filled, and if the tooth is badly damaged we plan a crown or a post and core.

Aftercare following root canal treatment
After root canal treatment the tooth may be more sensitive to pressure for a few days, especially when chewing. This is a normal tissue reaction that gradually passes within 3 to 7 days. Ordinary ibuprofen or paracetamol-based medicines are suitable for pain relief.
- For the first 24 hours avoid chewing on the treated side, especially hard food.
- If the tooth is only sealed temporarily, protect it so the filling does not fall out. If it does, contact the clinic.
- Continue your daily oral hygiene, using floss carefully.
- If you feel a worsening throbbing pain, swelling or fever, get in touch without delay.
After the final sealing it is important to protect the tooth from fracture. In most cases we recommend fitting a crown, especially on the back teeth. Regular check-ups every 6 months help us monitor how the canals are healing: we usually take a control X-ray 6 to 12 months after treatment.

Root canal treatment is, I think, one of the most difficult areas of dentistry. And there are plenty of challenges here.
For example, a patient often comes in with an aching tooth, having not slept for several nights, exhausted, full of painkillers, hurting so much that their head feels ready to split. Or the opposite: the person feels nothing, everything is fine for them, but the X-ray shows an abscess quietly dozing away. It needs treating, but the person does not want it, after all it does not hurt. Ah, all I can do is sigh.
It is at least good that these days we already have the means to help a patient effectively, but the treatment is expensive, takes a long time and needs more than one visit.
In canal treatment thoroughness matters enormously; you cannot rush it, the work must be done to within a fraction of a millimetre. Sometimes it feels as if you scrape and scrape at those canals, simply praying as you lean over the little tooth, yet there is no end in sight. Especially when endodontic retreatment is needed. The things we find in those canals.
Our colleagues in the past did not have the possibilities we have today, so very often we have to retreat teeth that were treated long ago, in order to save them from extraction. But sadly it does not always work; there are many reasons, and I will not go into them.
That is why in many European countries a practice is already taking shape: 'oh, why bother, out it comes, in goes an implant and hurrah.' But in my opinion this is not the most correct path; no implant will replace your own tooth root. So whenever there is a chance to save a tooth, I invite you to choose the harder road.
I was delighted when we acquired rotary instruments for cleaning canals. Life in the world of endodontics brightened a little, the work became far more efficient, quicker and even better in quality. Yes, perhaps the price does not look cheering to the patient, but it is justified: the instruments are expensive, you use the fine files a few times and throw them away. But I could no longer work the way I did at the beginning, ten years ago.
In the clinic we try to work as a team. And ever since graduating from Vilnius University I have purposefully gone deeper into endodontics, attended courses, kept improving and chased after all sorts of news, so root canal treatment often rests on my shoulders.
